You should contact either your broker or the company’s registrars, Equiniti Limited. You can access information regarding your shareholding by registering a portfolio at www.shareview.co.uk.
If the value of your lost, stolen, or destroyed share certificate is below £100,000, Equiniti, the registrar can arrange for the issue of replacement certificates. Simply contact Equiniti as soon as you can, quoting your shareholder reference number (SRN) if possible, and request a letter of indemnity. You will also need to pay an administration fee and a countersignature fee (which will be dependent on the value of the lost share certificate) to receive a replacement share certificate.
If the value of the missing share certificate is greater than £100,000 you will need to source bespoke indemnity arrangements directly with an appropriate insurance provider before a replacement share certificate will be issued.
You should write to Equiniti, sending a certified copy of the death certificate, together with the original Grant of Probate (or a sealed office copy), or the Letters of Administration.
Alternatively, you can call Equiniti, to report the death of a shareholder and speak with an agent directly to discuss the steps required.
The name(s) of the deceased person(s) personal representative(s) will be put on the share register while the estate is finalised. Equiniti will then place a temporary marker on the register to ensure that wherever possible, they do not issue further dividend payments or other correspondence in the shareholder's name. They will also send you the forms you need to complete.

Equiniti Limited provide an online service at www.shareview.co.uk that enables shareholders to register electronic communication and receive shareholder mailings by email. Once you have registered with the share portfolio service you can then select your preferred option for company mailings: email or post. To change your name on Dunelm’s records, please send documents confirming this change (for example, your marriage certificate or change of name deed) to Equiniti.
To transfer ordinary shares held by share certificate, please download and complete a Stock Transfer Form and return it to Equiniti.

If you are unable to attend the AGM and your shares are registered in your own name, we will send you a proxy form. On the form you should indicate whether you want to vote for or against each of the resolutions due to be decided for the meeting and return the form to the registrars using the instructions provided.
Alternatively, you may name someone else to go the meeting and vote for you as your proxy.
If your shares are registered in the name of your nominee, the nominee will receive the proxy form and you will need to contact your nominee to state your intention of attending ahead of the AGM.
